2. TO ENROLL IN COLLEGE HOCKEY SPOTLIGHT YOU WILL NEED TO COMPLETE 2 STEPS AS OUTLINED BELOW:

STEP 1:

The College Hockey Spotlight Itinerary, Waiver and Payment Instructions are provided below. If you HAVE NOT paid by credit card please follow the instructions and deposit your registration fee as indicated within the Payment Instructions below. If you are a Canadian Player, please follow the Canadian Deposit Instructions. If you are an American Player, please follow the American Deposit Instructions.

STEP 2:

Please review, sign and submit the ASSUMPTION OF RISK, WAIVER, INDEMNITY AND RELEASE FROM LIABILITY AGREEMENT (“Waiver”). Your waiver will need to be completed and submitted via email (both pages) to questions@collegehockeyspotlight.com before you can take part in ANY of the activities.

Players will not be permitted to take part in College Hockey Spotlight, or participate in any activities of the camp without having paid their registration fee and have agreed to, signed and submitted their Waiver form.

College Hockey Spotlight Policies

Refund Policy

College Hockey Spotlight DOES NOT issue refunds. If you cannot attend College Hockey Spotlight in the year of your registration for any reason, a CREDIT will be available and must be used toward registration within one-year of the date issued or the credit will become invalid. Exceptions to this policy will not be made for any reason, including weather, personal emergencies or circumstances, municipal/government policies or otherwise.

COVID Protocol

College Hockey Spotlight events will abide by the COVID-19 rules and regulations set by the relevant authorities within the jurisdiction of the event.

ASSUMPTION OF RISK, WAIVER, INDEMNITY AND RELEASE FROM LIABILITY AGREEMENT

For and in consideration of Player’s participation with College Hockey Spotlight, practices, games, program and/or on ice instruction, the Player and Guardian relinquish any and all liability for and cause of action for personal injury, property damage or wrongful death occurring to Player arising out of participation in such an activity.

Player and Guardian acknowledge, understand and assume all risks relating to ice hockey, on ice and off-ice training, and understand that ice hockey and training involve foreseeable and unforeseeable inherent risks, hazards and dangers to Player’s person including bodily injury, partial or total disability, paralysis and death, and damages, that no amount of care, caution or expertise can eliminate, which may arise therefrom and that Player and Guardian have full knowledge of said risks. These risks and dangers may be caused by the negligence of the Player or the negligence of others, including College Hockey Spotlight.

It is further acknowledged that there may be risks and dangers not known to College Hockey Spotlight or are not reasonably foreseeable at this time. Player and Guardian acknowledge, understand and assume all risks, if any, arising from the conditions and use of ice hockey rinks and related premises, acknowledges and understands that included within the scope of this waiver and release is any cause of action, arising from the performance, or failure to perform maintenance, inspection, supervision or control of said areas and for the failure to warn of dangerous conditions existing at said rinks, for negligent selection of certain locations for instruction, or negligent supervision of instruction by College Hockey Spotlight.

Player and Guardian agree if any claim for Player’s personal injury or wrongful death is commenced against College Hockey Spotlight, Player and Guardian shall defend, indemnify, and hold harmless College Hockey Spotlight from any and all claims or causes of action by whomever or wherever made or presented for Player’s personal injuries, property damage, or

wrongful death. This release, waiver and indemnity agreement is intended to be as broad and inclusive as is permitted by law of the Province of Ontario and if any portion thereof is held invalid, the balance shall, notwithstanding, continue in full legal force and effect. This Agreement shall be governed by and construed in accordance with the laws of the Province of Ontario and the laws of Canada applicable therein and the parties attorn to the Courts of the Province of Ontario. This Agreement shall be affective and binding upon the heirs, next of kin, executors, administrators, and assigns, in the event of the death or incapacity of the Player.

Player and Guardian acknowledge that they have been provided and have read the above release and have not relied upon any representations of College Hockey Spotlight, that they are fully advised of the potential dangers of ice hockey, on ice and off ice instruction and understand these waivers and releases are necessary to permit College Hockey Spotlight to provide the requested instruction.

Player and Guardian expressly acknowledge that they have had an opportunity to consult with and receive legal advice concerning all portions of this Agreement and has utilized this opportunity or has waived the right to do so. In executing this agreement, Player and Guardian acknowledge that they have freely and voluntarily executed this Agreement, understand their obligations under the Agreement and that they are waiving certain rights enumerated in this Agreement.
